Seeking a Licensed Clinical Social Worker for an opportunity at a dynamic clinic in Largo, Florida.

You will be focused on delivering comprehensive, patient-centered care across the health care continuum, as a Licensed Clinical Social Worker you will work closely with primary care physicians to deliver integrated member care, clinical coordination, coaching, consultation, and intervention with a particular focus on patients with a need for placement and care transitions and patients with behavioral health needs which require referrals and follow-up.

Essential Functions

• Conducting psycho-social assessments and provide psycho-social treatment to a wide variety of individuals from various socio-economic, cultural, ethnic, educational, and other diversified backgrounds.
• Providing psychotherapy or counseling to individuals, groups, couples, or families.
• Identifying symptoms and signs of psychiatric disorders; comprehensively assess, diagnose and treat symptoms.
• Preparing clinical reports in electronic health record to include diagnoses, prognoses, and treatment plans.
• Implementing and provide new treatment, training and rehabilitation programs.
• Monitoring, modifying and supporting care plans by evaluating patient's progress in setting and meeting established goals and revise their plans accordingly with a focus on educating and reinforcing appropriate behavior with the patient while at the same time promoting clear communication among the care team and treating clinicians ensuring awareness of and support for patient care strategies.
• Providing consultation services to other staff about the psycho-social needs of patients and the impact of psycho-social problems on health care and compliance with treatment.
• Cultivating a strong, cohesive, team-oriented relationship with practice partners at the local level, including on-site and remote interaction where appropriate, whereby the practice’s care team considers the Social Worker as an extension of and integral part of the practice’s care delivery program.
• Screening, advocating, guiding and intervening on behalf of patients, their family and/or care givers, in concert with the PCP, to identify opportunities for health services, modifiable risk factors and other support services.
• The position will apply data driven methods for identifying, screening and assessing patient's needs, including the identification and risk stratification of complex, co-morbid patients, with the objective of focusing care management efforts on the segment of the population requiring the highest degree of support with a spotlight on care transitions and behavioral health needs.
• Interpreting and integrating data to determine appropriate diagnostic and therapeutic procedures needed. Recommends community resources to meet patient and family needs.
• Triaging patient telephone calls and provides consultation.

Job Requirements

• Florida Licensed Medical Social Worker (LCSW) or able to obtain upon hire

Required Experience

• 3+ years related work experience

Required Education

• Masters Degree in Social Work required
