Responsible for reviewing and abstracting medical records for the annual
Assist with ensuring that the medical record collection, review
and abstractions are complete and compliant in accordance with NCQA HEDIS
technical specifications and guidelines
Reviews and integrates medical
information from electronic and paper medical records in a timely manner
Participates in medical record collection, requests and abstracts appropriate
assessing for assigned categories as needed

Prepare probationary and annual performance evaluations for
paraprofessional staff, Conduct disciplinary sessions with paraprofessionals
and contact HR and/or Director of Patient Services (DPS) for assistance as
needed, Assign and monitor the progress of RN assessments, Participates in weekly in-service session
reports, Complete daily report and submit to DPS, Prepare all discharges
for respective caseload so that DPS and RNs can discharge clients, Assist
patients and family members with ordering special equipment for the patients
